Application to Join Paws Squad Agility Club

The application to join Paws Ahead is an editable PDF file. There are two ways to complete and submit the form.

Method 1

After opening the PDF file, fill in the fields from within Adobe Reader. After all the information has been entered, click the submit button. Acrobat Reader will attempt to e-mail the form data to the Paws Ahead club secretary.

Method 2

After opening the PDF file, print the form, and fill the form in by hand and mail or return the form to a Paws Ahead member.
